A Weaker Sweden?

With an early PoD, say the 12th century, how could the power of Sweden be greatly diminished? Sweden reigned for many centuries as the predominant power in the Baltic, only being eventually surpassed by the Russians during the Great Northern War. But what if Sweden never achieved that predominance in the first place? How could this occur?
 
I don't think you have to go that long. Replace Oxenstierna with someone who has no talent or is a royal sycophant and you've done the lion's share of the work right there. Hell, here's a thought, have Oxenstierna switch his allegiance to the Danes and get them to lead the 30 years war. Another possibility might be a route at Brunkeberg somehow.

Not hard at all. Just keep Denmark in Skåne and dominant in the area, its utterly feasable to have Denmark and Norway nibble other borderlands away too.

Hell...If we're going back to the 12th century...
Finland wasn't even properly conquered then you could well have it becoming a nation of its own early.

Easy. You dont even need an early POD.

Just have a few days of the winter of 1658 2-3 degrees warmer than OTL!

No freezing of the Belts, no Treaty of Roskilde, no Danish loss of Skåne, Denmark-Norway and Sweden on more even terms in the long run! :)
