So what's the country called, then? "The United Kingdom of Great Britain, Northern Ireland and Sierra Leone"?
Not so sure about this. The original United Kingdoms were the Kingdom of Scotland uniting with the Kingdom of England then these two uniting with the Kingdom of Ireland however Sierra Leone does not bring her crown & Royal Seed. Perhaps just the UK and treating Sierra Leone something akin to Wales?
 
Only problem with that is that England, Wales and Scotland are all in Britain. Geographically, I mean. Sierra Leone isn't, so including it in "Great Britain" is a bit... off.

Maybe that's the way they would've done it at the height of Empire or something but today people probably wouldn't stand for it.

Hmm... wouldn't the plural indicate separate countries?
 
Well, the Queen has the title of 'Queen of Canada' already. It's likely that the first order of business sees Sierra Leone become a Commonwealth Realm while they sort out the details of the union (thus we have 'Queen of Sierra Leone'. Theoretically they might at this point be referred to as the Kingdom of Sierra Leone, though Commonwealth is more usual.

It's generally considered bad manners to perform thread-necromancy on threads more than a year old unless you have some really good reason to.
