A literal mafia state?

Due to their supporting of drugs and other illicit things, Transnistria has often been called a mafia state. That got me thinking, was there ever a chance or opportunity for the actual Mafia to control a country? The notion of a president Al Capone is a funny one to say the least.

The Mafia basically controlled Sicily (and much of Italy) for a long time. They bought votes and killed those who opposed them. As a result many of the politicians of Sicily (including a few Presidents) were bought and paid for. The only difference was that they didn't decided policy. As long as you let them do their business unimpeded they worked for you.

North Korea has been called "The Tony Soprano of Asia" with good reason. They are one of the biggest counterfeiters in the world, they have industrial scale meth labs and opium fields, they sell weapons to anyone with money, and even the diplomats are involved in smuggling drugs (diplomatic immunity means you can't search their diplomatic pouches, making them the perfect mules). Also the Kim family itself is incredibly mafia-like, from the strict organization with one man at the top to the violence that they display to anyone who goes against the family. In fact I created this image to illustrate this point:

 
What's the difference between your average Warsaw Pact "communist" regime, where only lip service is paid to the discredited communist ideology, where unfairly aquired wealth is the only incentive the party can provide to its members, where opposition figures routinely experience unfortunate "accidents" or suddenly "commit suicide", and a mafia?

Their institutions were too strong and personal loyalties not important enough for a proper mafia state.
 
Kosovo stood good chances at some time in the recent past. Albania, too. Montenegro has been called a mafia state. Panama at the time of Noriega could be described as a drug cartel state.
 
There were plans to make Sicily an independent state after the second World War. Some of which were backed by the Mafia itself.
