For USSR this would certainly be an Armageddon. Also for Europe, but USA would be to large extent spared the direct effects of the war. Soviets did not have large amount of missiles at that particular moment, so I presume the eventual exchange would be one sided.

However it would make for an 'interesting' world. I'd like to read such a TL.

I suggest you read The Cuban Missile War TL, it deals with just such a possibility.
 
Beyond the sheer destruction, psychologically you have an entire surviving generation which is mentally scarred and fucked up. The pro-"atomic war as an acceptable possibility" people seemed to have had this crazed idea that even if millions and millions of people were killed in an instant, with everyone personally knowing scores of people who died, and being forced to carry on rebuilding in the aftermath and just living in the aftermath, they'd still be just fine and happy.
