Is there any case where a 20th century ruler could have adopted “Ruler of All Humanity” as a formal title!

For that, you would need an universalist agressive nation aiming at world conquest, while having a strong leader principle.
Stalin can't do that, because he's gone Socialism in One Country.
A Trotsky USSR couldn't have a "Ruler".
Maybe a far-right or far-left France or USA that gets sufficiently close has a better shot.
 
Nothing short of a apocalyptic external threat will unite humanity.
Therefore, Ruler of All Humanity is impossible as a title, unless literal ASBs show up.
 
I could see Jean-Bédel Bokassa or Idi Amin declaring it as part of a propaganda campaign if they were any crazier than they were in OTL. Hell, Amin actually did something similar in OTL, so there's some precedent, even if it's absolutely insane. To quote Wikipedia:
After the United Kingdom broke off all diplomatic relations with his regime in 1977, Amin declared that he had defeated the British, and he conferred on himself the decoration of CBE (Conqueror of the British Empire). His full self-bestowed title ultimately became: "His Excellency, President for Life, Field Marshal Al Hadji Doctor Idi Amin Dada, VC, DSO, MC, CBE, Lord of All the Beasts of the Earth and Fishes of the Seas and Conqueror of the British Empire in Africa in General and Uganda in Particular", in addition to his officially stated claim of being the uncrowned King of Scotland.[98]
 
Sounds like a title better suited for the Pre-1900 board: I can see something so grandiose being claimed by Phillip Hapsburg of Spain/Austria/Everything-West-of-Brazil/Also-This-Bit-Here.
Now, if it was claimed by some religious leader of one of those New Spiritualist movements then sure, it just doesn't carry much weight beyond the cult faithful.

Just have the an Iranian Shah chose a different tack in reviving ancient Persian titles, and you could get fairly close. IOTL the Pahlavis went for emphasising the nationalist element (and therefore projected a lot of reinterpreted Sasanid stuff onto the Achaemenids) but the actual Achaemenid titling had a lot of universalistic claims (largely but not exclusively inherited from the earlier Sumero-Akkadian tradition - the Avesta sounds like it has enough universalistic material by itself - but then whatever had been Avesta by Achaemenid times is not very clear).
