How the heck is New England so screwed up in A.D. 2018? How did the theocratic Handmaid's Tale-esque regime make it so long?
The fact that the regime is still in place can be mainly attributed to the fact that the Theocratic Revolution of 1845 resulted in a government that was even more moderate in comparison to the modern one, and that the country only moved further backward as the rest of the world moved forward. The government of Jerry Falwell, established in 1970, led to more orthodox interpretation of scripture and law as Falwell's government began enforcing codes and religious practices that had not been taken seriously since the very early 20th century. Upon Falwell's death in 2007, Cal Thomas took over and continued his predecessor's reinsitution of radical orthodoxy. The country's current treatment of women was put to code with the 2009 Canonical Ruling on the Role of the Sexes, which served as an essential redrafting of laws that had existed under the original government of the People's Theocratic State of New England.

Currently, the state is considered to be internally unstable, and underground insurgencies have grown significantly since 2009. New York and Canada are hesitant to attempt to overthrow the government from the outside due to the possibility of an outward chemical or biological attack from New England, or even the possibility of a "governmental suicide," in which Thomas orders that all of New England's weapons be turned on its own population to prevent New Englanders from ever living under a free government or to stop foreign states from being able to use the land after the end of the Theocracy. At the rate its going, however, New England will destroy itself from the inside as the government and its secretive opposition become increasingly radical.

You mentioned New Brunswick and Nova Scotia as having been seized by New England in the 1840s. What has life been like since then?

All French Canadians who lived in the area were forced to either learn or speak only English, the only permitted language for use anywhere in New England. As the region had a higher number of Catholics than most places in New England, there were a high number of subsequent conversions to Protestantism. Any catholic that refused to convert would either be imprisoned until they did, or executed. Nova Scotia and New Brunswick have been largely homogenized like the rest of New England.

Both New England and the USAS use cults of personality. The USAS does it more, where all past heads of state are seen as being the pinnacle of human form. In New England, executives are treated with extreme respect as direct interpreters of the word of God.

It's strange that the south ended up with communism and the north ended up with theocracy, considering that IOTL it'd be the opposite.
 
New England kinda started out as a theocracy, the Puritans were trying to build a pure, Calvinist society in the New World, while the South (Virginia) began as a commercial enterprise, but nowadays the south is the religious region while the north is more secular.

However, New England sued for peace after New York failed to advance past north Massachusetts for several months. The terms were agreed upon in 1850: New York would annex Massachusetts, Rhode Island, and Connecticut in exchange for a guarantee of 50 years of peace from New York.
