What is the most dystopian AH work ever created?

The Drakaverse up until the Final War and its immediate aftermath is dystopic af, sure (though not for everyone). Drakon-era... might be argued as a dystopian utopia of sorts... It's firmly post-scarcity, nobody's sick, serfs live long and generally painless lives (servus are engineered for that, long healthy lives then a quick, easy aging and natural death). Most servus on Earth probably rarely interact with Drakas "in the flesh", those we see in the novel are living by themselves and Gwen's visit is treated as a rare occurrence worthy of celebration (yeah...). All in all it's dystopic in that people are engineered to be happy, but happy they are.
Might suck more for the rare "feral" homo sapiens who aren't living in nature preserves (like the Pygmies) and are occasionally hunted by some thrill-seeking Draka armed with a flint-spear "for sport", though I suspect the various fantasy species like orcs and goblins (the latter Gwen describes as so annoying that creating them was a mistake) would be more of a daily nuisance...
 
I'd have to go with WMIT for timelines on this site; it just keeps getting crazier and it's nowhere near done. For published works, probably the Drakaverse. Would George Romero's Living Dead series count as an honorary example? Humanity is virtually destroyed by Day of the Dead. The Dawn of the Dead Remake would have had society collapse even faster.

Does 1984 count as alternate or speculative history?
I tend to think 1984 more as the latter (if it's even an accurate description of what the novel is besides a dystopia) because we don't really know whether the history outlined in the novel is even true because of how unreliable Winston is as a narrator and the modus operandi of Big Brother and the Party.

I think he was trying to make them even more absurdly evil than they already were, but due to how evil Nazism really is he resorted to low blows
 
Nah it sounds like he just wanted shock value without actually understanding Nazism or anything around it. For a good example of making Nazism more evil than it was you can look at Himmler's ideas of paganism and the Burgundian System in The New Order.
 
The novel also depicts the Nazis as so flamingly homosexual that they decorate their homes with posters of attractive athletes with giant erections, and they're also big fans of hurtcore pedophile brothels where little girls are sent by their parents so sadistic pedos can torture them with whips and giant dildos.

Anyone remember “Christian Nation” by Frederic C. Rich? It was written to be a big jab at John McCain’s decision to pick Sarah Palin as his Vice President and how having an ultra-evangelical Christian in the White House to be a very bad idea. In this story McCain became President but died suddenly and succeeded by Palin. Under Palin, she gradually enforced her religious upbringing and turning the US into a Christian theocracy, which then her more extreme successor led in crushing secular Americans in a second civil war, persecuting the homosexuals (legally murdering them or sending to “reorientation camps”), making other non-Christian religions illegal, destroying books that are labeled “satanic”, Fox News is (unsurprisingly) turned into a government propaganda machine, the internet and telephones are constantly monitored by the Purity Network (sponsored by Apple!), and among others sex-outside-of-marriage is also punishable under law.

But this theocracy has severely diminished the US in global affairs and turning inward, allowing Russia to expand its influence over Europe and as does China with Asia and Australia. Islamist groups also take control of all of Africa and Israel similarly falls under a Jewish theocracy.

So yeah the book is basically a jab at Sarah Palin and American evangelicals who took their faith too far from reading Left Behind.
 
Pax Atomica: An American Dystopia is also terrible and heavily dystopic, with the world is stagnating culturally and technologically (partially). US-led “Free World” becomes Madness-esque ax-crazy, going around bludgeoning people they deem undesirable, and the Socialist Bloc is full-blown Orwellian. And the fact that it ends in nuclear warfare adds in the total suffering.
 
Decades of Darkness isn't particularly dystopian by OTL standards. Almost everywhere in that world outside of Western Europe and the Americas is better off than OTL, or at least no worse. Eastern Europe and Asia in particular are better than their mid-20th century OTL equivalents. That's one of the delicious ironies about DoD.
