Well, he was already Protector of the Rhine Confederation IOTL, in addition to being Emperor of the French and King of Italy. But of course, thats not strictly a monarchical title. In any case, I dont think Napoleon crowning himself HRE would have fit to his ideology - for one, it was a title of the old order, and in many ways Napoleon wanted to shape a new monarchy and a new nobility. And more importantly, by then the title was very much fixed to Germany, and Napoleon very much wanted France to be the centre of his Empire.
