First concern - over what? Have the two countries ever faced one another down over some major issue?
That's a good starting point if so.
 
- Iranian Coup in 1953 not only fails, but backfires horribly. Iran remains democratic and starts leaning towards the Soviets.

- India becomes friendly towards Iran (as both are democratic and vaguely pro-Soviet)

- Iran attacks Pakistan (vaguely pro-US) during the 1965 Indo-Pakistani War, in order to help India.
 
No 9/11. The West doesn't go into Afghanistan. Iran and Pakistan both intervene there in support of favoured factions (and , at least in Iran's case, historical claims to overlordship) and the situation escalates...

The 1998 Crisis between Iran and the Taliban could well have led to a war between Pakistan and Iran.

So Taliban is basically Pakistan-supported?
(why does the US consider Pakistan to be an ally, oh well that's another question)
What was the OTL reason the crisis didn't lead to war?
