Assumption: Butterflies stay small till first time a Vice President of a second term President is elected President...then the President gets a third term (yeah, I'm rolling my eyes, too). Result: Ronald Reagen is re-elected in 1988. Analysis: IIRC, Reagan was visibly developing Alzheimer's Disease by 1992. One of the characteristics of the disease is a denial of the condition by the sufferer (believe me, I know). So we have a President who is showing an inability to discharge the powers and duties of his office who will not admit that. NOW WHAT!?
 
If the media noticed Reagan's condition might there be a situation of difficulty for George Bush Senior. The power under the 25th amendment requires the Vice President to sign the relevant paper.

The extreme version is that a Vice President is threatened with Impeachment for NOT invoking the 25th.

BTW if it came to that there is the odd fact that in theory a VP would preside over his own Senate trial following impeachment by the House
