It depends what you mean by 'worse.' If you mean by more nations getting involved, have the United Kingdom attempt to broker a cease-fire and get caught in between the two parties. If you mean by more deaths, have the French use the milletruse in battle, then have a long siege of Paris and another of Metz. Real possibilites for disease and plague in those two.
