The obvious answer is less influence of Rasputin. However, I believe Rasputin was not the first mystical character of religious nutjobbedness that Nicholas and Alexandra fell in with, and its always possible that they will take up with Rasputin to some extent or other, or perhaps more likely with another such undesirable, with different causes but possibly the same effect.
Alexei would have been seen in public more often, and as an honorary officer would have reviewed troops alongside his father and be better known, better loved perhaps.
I have often wondered what if Alexei (or equivalent) had been born in the place in time occupied by one of his older sisters - probably not the younger two as he'd still be too young in 1914+ to make a real difference, but if he had been legally an adult when the troubles began...even if a hamoephiliac

Russia would still be too weak and would be acting like a stronger nation then it was. Russia would still be had the shit kicked out of it by the Gerys in WW1 and the royal family would have been booted during a revolution. If the reds would have sized power, or maybe, Alexei develops an incredibly strong character and leads the whites as kind of figurehead...but i forgot how old he was...was he still a teenager? because this is a lot less likely the younger he gets.

Or maybe cosmic butterflies allow the Czar to escape and give the Whites a huge boost in strength, or maybe an independent Czarist state.
