In a similar vein you could have Richard of Cornwall (http://en.wikipedia.org/wiki/Richard,_Earl_of_Cornwall) becoming Holy Roman Emperor.

I was planning a timeline around this a few months ago, called the Platagenet Holy Roman Empire. It led to Richard being King of England and Sicily, along with ruling the HRE. It sort of fizzled out, although I plan to actually write it at some point in the future.
