A lot would depend on when exactly and what area and people are involved in the ISOT. [I.e. if there was a strong emperor and he and a good proportion of the army was in residence at the time it would make a good degree of difference].
However the city is in for a hell of a lot of difficulties even if under best conditions. Its just lost the rest of the empire which means its going to be very short of food very quickly. Coupled with being in a radically different environment and climate and the total chaos resulting. [While if it happens to be in hurricane season!] Not sure the Romans would even make contact with the Maya as their a good way south and the Romans would have to establish a stable state and expand from there to make contact. [Unless some butterflies from their appearance means the Mayans start moving wider afield].
The disease imbalance is probably a lot less as a number of the big killers that devastated the new world probably haven't developed yet. However at its height Rome is a hell of a big city with a lot of poor people and despite Roman attitudes to hygiene plenty of dirt so its bound to be a big infection source for the locals.
